We want to know that services provided at akt produce the right outcomes for you. These tools are used for you to measure your journey with us and help us reflect on the work we do together. Outcome measures and tools can help to show you and your caseworker where you are heading, where you have come from and what you are achieving. They are also a great way to see how things are going and how areas of a service can be improved. This is a self-assessment tool to be completed with the support of your caseworker at the beginning of your first keywork session and should complement your support plan. You will revisit this tool throughout and at the end of your journey with akt. You may also request to revisit this tool at any time to help evaluate how effective your current agreed goals are. If you have any questions regarding the use of these tools and how this data is used to shape your services, please don’t hesitate to ask your caseworker.
